篇名 Postoperative essential fatty acid deficiency in ptients with colorectal cancer

摘要 Patients with colorectal cancer are frequently malnourished. The incidence of essential fatty acid (EFA) deficiency however is not well documented. The aim of the study is to try to (1) detect the incidence of clinical or subclinical EFA deficiency in patients with coloretal cancer, (2) evaluate whether surgical trauma and a short term of postoperative fasting increases the incidence of EFA deficiency, (3) assess the effectiveness of infusion of long chain triglycerides (LCT) or a mixture of long and medium chain triglycerides (LCT/MCT) in reversing EFA deficiency. Nineth-nine patients with colorectal cancer undergoing colon resection were divided into five groups. Twenty patients (group I) did not receive parenteral nutrition for 5 days. Nineteen patients (groupII) received parenteral nutrition without fat emulsion for 5 days. Twenty-one patients (groupIII) received parenteral nutrition, and 20% LCT fat emulsion for 5 days. Eighteen patients (group IV) received parenteral nutrition and a mixture of 20% LCT/MCT fat emulsion for 5 days. Twenty-one patients (groupV) received parenteral nutrition without fat emulsion for a period of two to six weeks. All the patients fasted during the study period. An eicosatrienoic acid: arachidonic acid ratio of greater than 0.4 was defined as evidence of EFA deficiency. It was uncommon for patients with colorectal cancer to have fatty acid deficiency prior to surgery. Prolonged fasting with parenteral nutrition increased the chance of essential fatty acid deficiency. Infusion of LCT or a mixture of LCT and MCT fat emulsion completely eradicated essential fatty acid deficiency.
