Blunt cardiac rupture is associated with a very high rate of mortality. A 35-year old woman of a car accident on the highway suffered from semi-coma. Anterior chest tenderness and unstable blood pressure were noted on arrival in our emergency department. Suspected cardiac tamponade was proved by echocardiography. Surgery was performed after pericardiocentesis failed. Through a median sternotomy after initial subxiphoid pericardial window with massive dranage of blood, reptire of the right atrium was found and repaired. The postoperative course was uneventful, and the patient recovered. In a high indux of suspicion of blunt cardiac rupture, the use of echocardiography in the emergency department can quickly and noninvasively document this problem. Using our experience in this case and those in the literature, we recommend a workup flowchart for blunt cardiac rupture. Rapid diagnosis of this disorder, avoidance of unnucessary procedures and prompt surgical intervention will enhance the survival of these patients. |
