Spontaneous focal perforation of duodenum in premature infant: A case report

Focal duodenal perforation in infants of very low birth weight is uncommon, and its occurrence in the fourth portion of the duodenum has rarely been described. We report a case of ‘spontaneous’ perforation in the fourth portion of the duodenum of a premature infant. Such “spontaneous” perforations may involve a multifactorial etiology. Possible factors are discussed. Abdominal distension is often observed since it develops abruptly and progresses rapidly. Blue-black discoloration of the abdominal skin is an important clinical finding for gastrointestinal perforation. The patietn was treated with primary closure for the duodenal perforation. Two months later, the patient recovered from surgery and remained asymptomatic during a follow-up period of six months.
