Strongyloides stercoralis (S. stercoralis) is an excreta-transmitted nematode that is most prevalent in tropical and subtropical regions, but has rarely been reported in Taiwan. Clinical symptoms associated with S. stercoralis infection include diarrhea, abdominal pain, anorexia, weight loss, nausea and vomiting. A 61-year old male who complained of abdominal distention and progressive abdominal pain for one week was admitted to our hospital. Peritonitis developed on the second hospital day. An emergent exploratory laparotomy was performed with bowel decompression. Unfortunately, ileus persisted postoperatively. After 38 days of conservation treatment, the patient contiuned to have abdominal distention and apin, nausea and vomiting. Therefore, he underwent a repeated laparotomy under the impression of adhesion ileus. Enterolysis and jejunal biopsy were performed. The pathology showed infection with S. stercoralis. The patient was treated with albendazole (Zentel) 400 mg per day for 5 days. All of his symptoms resolved themselves and he was discharged 3 days later without any further problem. |
