For young patients with symptomatic spondylolysis, direct repair of a pars interarticularis defect is ofen attempted to fix the defect and preserve the motion segment of lumbar-column segment. If symptomatic spondylolysis is combined with spinal bifida, pedicle screws instrumentation with posterolateral fusion is considered in most cases. We extended the use of direct repair of the prs defect and used cerclage-wiring of the bifida defect with bone-grafting for these patients, to try to preserve segment motion.
From 1994 to 1998, 15 patients with spondylolysis and spinal bifida occulta were treated using this technique at our hospital after conservative treatment had failed. Thirteen of these patients underwent a bilateral solid union with good to excellent results, while the implant failed in two of the cases with a non-union of the defect, results assessed as fair and poor. We recomment this technique for young patients as the motion of the segment can be preserved and adjacent disc degeneration can be avoided. |
