The esophageal anastomosis: What lessons have we learned?

Anastomotic leakage between the esophagus and the conduit used for esophageal replacement is the highest among any surgical anastomosis, and is a dreaded complication because of its consequence. It remains a principle cause of surgical sepsis and its associated morbidity and mortality is high. Even if the patient survives, hospital stay is prolonged with its associated increased cost. The incidence of this complication varies widely. A review of surgical series reported in the 1980s revealed an average leakage rate of 12%. But in some relatively recent reports, leak rates of up to 30% are still reported. Even in specialized centers, rates of around 10% is seen.
