Pneumoperitoneum caused by spontaneous rupture of pyogenic liver abscess: Case report

To describe an unusual case of spontaneous pneumoperitoneum secondary to the rupture of liver abscess caused by Klebsiella pneumoniae (K. pneumoniae) in a diabetic ketoacidosis patient. Case report: A 46-year-old woman was hospitalized due to dyspepsia and pyrexia off and on for one and half months. Pneumoperitoneum was noted on plain abdominal and upright chest X-ray film in the following morning. She was treated successfully by laparotomy with multiple drainages and antibiotics. Bacterial culture revealed K. pneumoniae, mostly found in Asia, as contrast to E. Coli in Western countries. In acute abdominal pain with spontaneous pneumoperitoneum, rupture of pyogenic liver abscess induced by gasforming bacteria should be added to the differential diagnosis, especially in diabetic patients.
