In the past twenty years, vena cava filters were widely used as adjuncts to anticoagulation therapy for preventing a life-threatening pulmonary emolism. Complications of organ perforation due to struts penetration and device migration were described in the literature. However, structural failure that results in struts fracture and causes symptoms was quite rare. We present a 46-year-old male who had received pulmonary embolectomy and intraoperative Greenfield vena cava filter placement 26 years ago. While evaluating hia right upper quadrant abdominal pain, a fracturing of the struts with renal vein penetration and migration of the filter were found. He received an exploratory laparotomy with clipping and removal of the penetrating struts. The patient was free of abdominal pai after discharge. |
