Abdomunal colectomy as a treatment of obstinate slow transit constipation: clinical results and aetiological analysis |

Purpose: To assess the therapeutic effects of colectomy for obstinate slow transit constipation (STC), and to explore the etiology of STC.
Methods: Thirty four patients with STC, who underwent colectomy, were enrolled to be studied. The observed parameters included the velocity of colon transit, defecography, colonoscopy, the level of serum sex hormones, and immunohistochemistry. Following up investigation was conducted for 6 months to 8 years, 34 months in average.
Results: The colon transit time was ranged from 96 hours to 240 hours, with an average time of 136 hours. 85% (29/34) patients had obstructed outlet constioation (OOC). 50% (17/34) patients showed abnormal level of serum sex hormones. Immunohistochemistru showed that intestine had more nitric oxide synthase (NOS) and less vasoactive intestinal peptide (VIP) positive neuron. Most patients recovered well after operation, execpt for 5 cases (17.9%) of small bowel obstruction and 1 case (3.6%) of recurred constipation.
Conclusion: Colectomy is an effective apprach to treat slow transit constipation. The etiology may be related to abnormal sex hormones and enteric nervous transmitter disorder. |
