An accessory scrotum is an extremely rare congenital scrotal anomaly defined as the presence of scrotal skin outside of its proper location with no contained testicular tissue. We report a case of one-year old male who revealed an accessory scrotum located on the perineum accompanied by ventral chordee, penoscrotal transposition and a bifid (main) scrotum. There was no other congenital anomaly. His karyotype was 46XY. Surgical reconstruction comprising of chordee release, excision of the accessory scrotum, and penoscrotal repositioning was performed without difficulty. A satisfactory external genital appearance was achieved following surgery.
