Primary Hepatic Malignant Fibrous Histiocytoma with Chest Wall Invasion: Report of a Case |

Malignant fibrous histiocytoma is the most common soft tissue sarcoma of late adult life. This tumor tends to occur in the deep soft tissue of the extremities, but rarely develops in the liver. We report a patient who presented with chest wall tumor as the only initial symptom. He complained of right back pain with swelling in the right postero-lateral chest wall, which he had first noticed one month before. Physical examination and routine blood studies were performed, as well as abdominal ultrasound and computerized tomography of the thorax and abdomen. A poorly circumscribed hypodense mass lesion was found in the posterior segment of the liver, with direct invasion of the chest wall. Tissue biopsy of the chest wall tumor led to the diagnosis of malignant fibrous histiocytoma. The immunohistochemical study showed that the tumor cells were positive for CD-68 and vimentin stains, and negative for alpha-fetoprotein, TTF1, HMB45 and carcinoembryonic antigen (CEA).
The patient died two months after initial diagnosis. We suggest that primary hepatic malignant fibroid histiocytoma, though rare, should be considered in the differential diagnosis of liver tumor. Early and complete surgical removal of primary hepatic malignant fibroid histiocytoma is necessary because of the aggressive nature of the tumor. |
