Purpose: Two different techniques for multiple cadaveric organ procurement were published by Starzl et al. Since multiple organ procurement and transplantation are becoming increasingly popular in Taiwan, we hoped to establish a standard procurement technique for different organ procurement teams.
Materials and Methods: We analyzed 89 cadaveric donors, including 52 kidney-only donors and 37 multi-organ donors, harvested from August 1987 to December 2002. Harvesting techniques were reviewed in relation to posttransplant complications from the kidney transplantation point of view.
Results: Among 176 kidneys retrieved, six kidneys were discarded due to iatrogenic damage. Two cases of renal vein thrombosis and 4 cases of ureter complications were encountered in the recipients, which might be related to procurement procedures.
Conclusion: We suggest 1) preservation of aereolar tissue on the surface of the inferior vena cava and renal veins by both the liver and kidney teams, 2) sharp transaction of the inferior vena cava aorta by the liver team, 3) avoidance of vascular clamp application caudal to the superior mesenteric artery by the liver team, 4) avoidance of lateral traction of both kidneys large vessel dissection by the kidney team. |
