Gallbladder carcinoid tumor is a rare neoplasm and usually lacks specific
symptoms. Only 31 patients have been reported in the English literature to date.
We report herein a case of a 73-year-old woman complaining of epigastralgia with
nausea and vomiting who was diagnosed to have acute cholecystitis and underwent
emergent laparoscopic surgery initially. However, we converted to laparotomy for
en-bloc excision immediately because gallbladder malignancy with liver invasion
was suspected. Pathological diagnosis was malignant carcinoid tumor of the gallbladder with direct liver invasion. No recurrence or distant metastasis has been noted during follow-up for 5 months to date. We review the literature and discuss the diagnosis and treatment of such a rare neoplasm. It is suggested that early diagnosis, although difficult, and curative resection of the gallbladder carcinoid tumor are important. Moreover, if there is suspicion of gallbladder malignancy with liver invasion, conversion from a laparoscopic approach to an open procedure should be done without hesitation. |
