Background:Recently laparoscopic surgeries are more commonly performed and several studies reveal that they are a safe alternative to conventional open methods. Although most general surgeons are experienced in laparoscopic technique, some senior pediatric surgeons hesitate to learn this technique. There are many reports describing the advantages, disadvantages and different techniques of laparoscopic appendectomy (LA), but no report describes a learning course for LA. The authors present their experience in learning LA.
Methods:Few pediatric patients with acute appendicitis underwent LA at
Changhua Christian Hospital before July 2000. During the 13-month period
between July 2000 and July 2001, 113 children (< 18 years old) had appendectomies. They were divided into Group I (open appendectomy [OA], n=59) and Group II ( [LA], n=54). LA was not performed on patients with peritonitis or suspected perforated appendicitis.
Results:The groups did not differ in age, body weight, or gender. The mean operative time was similar between these two groups. Group I had longer hospital stays (p<0.01) and a higher complication rate (p<0.01) than Group II. After exclusion of perforated cases, there was no difference in operative time, and the difference in hospital stay became insignificant (p=0.067).
ConclusionsF:or selected patients, LA is the method of choice since it is a simple and easy-to-learn operative method. It is also the best and most basic laparoscopic technique for senior pediatric surgeons to learn. |
