摘要 |
Spontaneous mesenteric bleeding, referred to as mesenteric apoplexy, is a rare
acute abdominal surgical condition. A 75-year-old man had hypertension without
regular treatment. He presented as having had abrupt diffuse abdominal pain for 2 days at our emergency department (ED). He had no history of abdominal trauma
or severe cough. On physical examination, he was severely anemic with
hypotension (BP 76/49 mmHg). A rapid blood transfusion was given, and computed
tomography of abdomen showed a 10 cm iso-densed mass over the right abdomen.
Emergent laparotomy was carried out and about a 350 ml blood clot in the
mesentery of the proximal jejunum and 1200 ml of old blood in the abdomen were
found. The blood clot and intraperitoneal blood were evacuated. No further
bleeding spots could be found in the whole mesentery. The abdomen was closed, the patient recovered uneventfully and no further therapy was given. Then, he was symptom-free for 27 months. We report this rare case and review the literature. |
