Objective: To evaluate the arthroscopic treatment results of patients with knee joint
gouty arthritis.
Methods: Arthroscopic debridement and lavage surgery was performed on 28
knees of 18 patients between 1995 and 2006. The knee joint functions were evaluated
with the Lysholm knee rating system pre- and post-operatively, the Wilcoxon signedrank
test was employed to compare the results. The subjective knee form of the
International Knee Documentation Committee (IKDC) was used to assess the
current functions of the treated knee joints.
Results: Three patients were lost to follow-up, 1 died from unrelated causes, and 1
was incapacitated by geriatric diseases, and thus five patients were excluded.
Thirteen male patients (20 knees), aged 45.2 years on average, were followed up for
a mean of 71.2 months. Knee joint pain and effusions were the symptoms showing
greatest improvement post-operatively. Pre-operative knee joint flexion contracture
of more than 10 degrees did not improve post-operatively. Moderate to strenuous
sport activities were still limited at the last follow-up. The average Lysholm score
improved from pre-operative 51.9 to post-operative 80.5 (p=0.001). The average
IKDC score was 66.05, and a comparison of the patients’ results with those of the
normal, same gender and age group showed that no patient achieved a score above
50th percentile.
Conclusions: Arthroscopic debridement and lavage surgery of gouty arthritic knee
joints is an effective method in relieving knee joint pain and effusions. However, this
operative method probably cannot resolve knee joint contracture or increase the
patients’ sports abilities. |
