摘要 |
To achieve maximal tumor excision and to save neuro-function are the supreme goal
of neurosurgical practice. However, these two attempts are in conflict with each
other in most circumstances and need careful estimate and balancing. With regard
to this requirement, many advanced neuro-imaging procedures are focused on
illumination of the active functional area of the cortex and the crucial fiber tract to
implement in neuronavigation with anatomical imaging. Among the developing
modalities, diffusion tensor imaging (DTI) and functional magnetic resonance
imaging (fMRI) integrated with anatomical MRI have been most reliable in
functional preservation. In this article, we illustrate a case of grade II astrocytoma
involving Broca’s area and the internal capsule, treated under neuronavigation with
imaging fusion of fMRI, DTI and anatomical MRI. The speech and motor functions
were well preserved during the observation period of 12 months. Furthermore, there
was no relapse of tumor shown by MRI evaluation. For verbal function preservation
and maximal tumor excision, imaging fusion of fMRI and DTI in neuronavigation is
of great benefit for a tumor involving Broca’s area. |
