Transumbilical Extracorporeal Laparoscopic Appendectomy for Uncomplicated Pediatric Appendicitis |

Objective: To introduce a minimally invasive surgical procedure and to develop a
flowchart for uncomplicated pediatric appendicitis.
Methods: From April 2005 to December 2007, all pediatric appendectomies
followed an appendectomy flowchart designed by pediatric surgeons at our hospital.
Informed consent was secured in all patients. A newly developed technique called
transumbilical extracorporeal laparoscopic appendectomy (TELA) was used. The
procedure was performed through an umbilical incision using a laparoscope with a
working channel.
Results: A total of seventy-five pediatric appendectomies were performed at our
hospital. Fifty-one patients with uncomplicated appendicitis underwent the TELA
procedure. Thirty-four appendectomies (67%) were successfully performed in this
manner while the procedure failed in 17 cases (33%). Among those 17
appendectomies that failed, 7 (41%) were successfully converted into laparoscopic
appendectomies (LA) while another 10 appendectomies (59%) were converted into
open appendectomies (OA). The mean operation time for TELA was 78.7 minutes
(30-187) and the average length of stay of a TELA patient was 2.6 days. No serious
complications were encountered except for a wound infection in 2 patients who
underwent TELA.
Conclusions: Uncomplicated appendicitis in pediatric cases can be treated simply,
safely and successfully by using the TELA technique. The procedure has the benefit
of leaving an almost invisible umbilical scar, saving the expenses of trocars and
autosutures when compared with LA. |
