Patients receiving anticoagulants may develop spontaneous intramural hematoma in the bowel, with consequent intestinal obstruction. In the past, this entity called for much attention. Now, the incidence is very rare and it is easily missed by primary care physician . A 73-year-old male had received coumadin for two months following a Bentall operation. He suffered from progressive abdominal distention and intermittent cramping pain for two days. Mechanical ileus was suspected after evaluation and conservative treatment was given first. He received exploratory laparotomy due to persistent abdominal pain and rebounding pain. At laparotomy, a massive hemoperitoneum and an 80 cm dilated jejunum with severe swelling and hemorrhagic picture were found. Thus, intramural hematoma of jejunum due to coumadin was impressed, and no jejunal resection was performed. The patient was discharged smoothly after conservative treatment. It should be emphasized that this process is easily reversible, and that awareness of its existence is imperative to obviate unnecessary operative intervention. We also review the literature describing the clinical course, evolving diagnostic technique a nd management. |
