Temporalis Muscle Swelling Secondary to Chronic Mandibular Third Molar Pericoronitis

Swelling of the temporalis muscle is a rare clinical condition with only a few cases reported in the literature. A case of temporalis muscle swelling, manifesting as a temporal fossa mass secondary to chronic pericoronitis of the mandibular third molar, is described in this paper. A 20-year-old female patient complained of a painless mass located for two weeks in the left temporal fossa. CT scan through the region demonstrated diffuse swelling of the left temporalis muscle. Biopsy was done, and histology showed interstitial lymphocyte infiltration; this suggested an infectious process. An impacted left mandibular third molar with chronic pericoronitis was discovered thereafter. Successful treatment consisted of extraction of the tooth and systemic antibiotics given with complete resolution of the condition at three-month follow-up. No recurrence was noted at sevenmonth follow-up.
