Between 1077 and 1993, 11 patients with angiosarcoma of the head and neck were treated at National Taiwan University Hospital. Of the 6 males and 5 females, average age was 56 years, with a range of 24 to 80. Six lesions arose from the scalp; the remainder occurred in the soft tissue of the face and neck. Five patients presented with multifocal disease. In one case, angiosarcoma developed in previously traumatic skin. The overall prognosis was poor, with a two-year survival rate of 50% (5/10) and a five-year survival rate of 12%. The major modalities of treatment were surgery and radiation. Of the 11 patients, 3 have survived without recurrence and have lived for 69 months, 56 months and 41 months, respectively. Eight patients had recurrences after primary treatment, and two had pulmonary metastases. The tumors were poorly circumscribed and spread horizontally for a considerable distance, especially in the scalp. Based on this limited experience, radical surgical removal of the primary tumor alone was not adequate. A review of the literature suggests that surgery with lymph node clearance if possible, followed by wide, high-dose radiation, offers the best chance for control of disease. Further evaluation is needed of chemotherapy as an adjuvant agent. |
