From February 1991 to May 1993, 10 patients with acute displaced femoral neck fractures were treated with closed reduction and Knowles pinning, combined with vascularized iliac bone grafting. There were nine males and one female. Mean age of the patients was 46.5 years old, ranging from 29 to 59 years old. Location of the fractures was four subcapital and six transcervical. The mean time-interval from injury to surgery was 4. 7 days, ranging from 1 day to 10 days. One patient was excluded from this study because of early lost of reduction from poor compliance. The mean followup time of the remaining nine patients was 31.1 months, ranging from 24 to 40 months. All fractures united within four months. The average union time was 3.2 months. One patient who developed systemic lupus erythematosus one year after the fracture and required a high dose of corticosteroid had evidence of avascular necrosis in plain X-ray at the last follow-up. Freedom from pain and nearly full range of motion of the hips were noted in all the remaining patients. |
