One-Stage Reconstruction of Complex Hand Defects with Composite Free Arterialized Venous Flaps: A Preliminαry Clinicαl Result

Venous flap from the forearm can incorporate either a segment of cutaneous nerve or palmaris longus tendon. With microsurgical technique, this composite flap can be applied as a free arterialized venous flap to accomplish one-stage reconstruction for complex hand defects which involve not only soft tissue loss but also loss of the underlying nerve, artery or tendon. From 1991, five cases with such complex hand defects were reconstructed here by composite free arterialized venous flaps. There was no flap compromise. The esthetic and functional outcomes have also been satisfactory. This method further extends the application of venous flap from simple soft tissue coverage and is sui:,erior to conventional two-stage, or multiple procedures in one-stage reconstruction for this category of hand defects. This method is believed to be a primary choice for difficult hand reconstruction in this modern era. More liberal clinical application should be encouraged.
