Despite advances in trauma care in recent years, successful treatment of severe hepatic injuries remains a formidable challenge. Exsanguination remains a major cause of mortality. There are a variety of methods for treating devastating hepatic injuries, but none seems adequate. In view of the recently reported succcess in control of hemorrhage from, and salvage of, injured spleen and kidney with an absorbable mesh wrap, an attempt has been made here to use such mesh wrap in the management of severe hepatic parenchyma injuries. In the initial experience with three cases of severe liver injury, all have achieved hemorrhage control. Of these, two cases recovered well and one was complicated with intrahepatic abscess. However, all three cases have survived. A comparison between the technique of mesh-wrap hepatorrhaphy and the conventional method are discussed in detail in the text. |
