Overexpression of the tumor suppressor p53 was investigated by immunohistochemistry in 149 primary gastric cancer cases. Positive staining, regarded as an indication of gene mutations, was evident in 27 (18.1 %) of the tumors. No correlation was observed between expression of p53 and various clinicopathological factors, including age, sex, tumor site, gross type, tumor size, histologic type, depth of invasion, node metastases, distant metastases and TNM stage. However, 5 of the cancer cells in the 10 metastatic lymph nodes stained positively for p53 even in primary tumor negative patients, implying that gastric cancer with a mutant p53 has high metastatic potential and metastases to lymph nodes probably occur. Survival analysis revealed that for patients with p53-expressing tumors, the prediction was worse than those with non-p53 expressing tumors (p=0.078). Further analysis revealed a significant survival difference for diffuse type gastric cancer but not for the intestinal type. These findings indicate that, in the case of human gastric cancer, overexpression of p53 is associated with the metastatic potential of gastric cancer and the presence or absence of p53 may serve as a prognostic indicator in patients of diffuse type gastric cancer. |
