The failur已rate for repair of primary inguinal hernia remains unacceptably high. Repair of recurrent inguinal hernia is an ubiquitous and troublesome problem. The conventional anterior approach has a high recurrence and complication rate. Preperitoneal prosthetic herniorrhaphy can prevent dissection
of the cicartrixed cord, and the inguinal floor can be reinforced with an unabsorbable mesh. During the post 4 years, 152 herniorrhaphies were undertaken in 137 patients at this hospital. Of those 126 (83%) of them were available for follow-up examination. The mean follow-up interval was 26 (8-49) months. Of the 3 (2.4%) recurrences noted, all occurred because of technical error. There was scrotal swelling in 3 (2%) cases, and 3 (2%) cases had hematoma. Those with hematoma had severe adhesions because of previous operation. No infection occurred in this series. Preperitoneal prosthetic herniorrhaphy should be a good choice for recurrent inguinal hernia, but more training and much experience are needed for total success. |
