This is a retrospective study of a six-year experience of modified CondonNichols
bowel preparation. Only patients with colorectal malignancies who
received bowel content culture were analysed. Excluded from the study
were patients with emergency surgery, colon obstruction, colon perforation
and peritonitis. Three hundred and sixty-two patients underwent one-day
bowel preparation with clear liquid diet, laxatives, oral antibiotics (erythromycin
and neomycin) and enemas. Cultures were taken after bowels were
transected. The cultures were reposted as “no growth” in 186 patients (51.
38%); 176 patients had different types of bacteria cultured, with one type of
bacterium most frequently seen (87 patients). No bacteria were cultured in
53.33% of the well-prepared colon. Only 11. 76% of the poorly-prepared colon
showed negative culture. This series supports that vigorous mechanical
cleansing with appropriate oral antiseptics can sterilize colon in half of the
cases treated. However, different bacteria are still present in the colon in
half of all cases. Meticulous handling of the bowel and careful avoidance of
contamination are still a gold standard to decrease postoperative infection
βfter colorectal surgery. |
