A case is reported of an accidental penetrating injury of the chest wall and
neck with a dirty steel rod which remained in the torso. Fortunately, the patient had no injury of intrathoracic great vessels, and was fortunate to
have then undergone appropriate management. Emergency thoracostomy tube drainage to overcome open pneumothorax and establishment of an airway with the help of the flexible bronchoscope to exclude any tracheobronchial injuries were performed, followed by careful neck exploration to remove the rod without damaging vital organs. However, the postoperative course was eventful, complicated by mediastinitis, the development of pneumonia and subsequent adult respiratory distress syndrome.
The patient finally survived and assumed normal daily activities with
no sequelae at follow-up. Special attention should be paid to initial resus-
citation and postoperative intensive care in dealing with the surgical and
medical problems encountered in this case. |
