From September 1993 to December 1994, four patients with large defects of the femoral shaft were treated with "double-barreled" free vascularized fibular grafts, in addition to internal or external fracture fixation. There were two females and two males whose average age was 41, ranging from 30 to 54 years old. Location of the defects was in the middle third of the femoral shaft in three patients, and in the lower third in one. Three of the lesions were infected nonunions. All of the patients had received multiple operations before the bone transfer. Lengths of the bony defects were centimeters in average, ranged from 6 to 11 centimeters. The mean follow-up period was 16 months. Three of the patients achieved solid union of the fracture within four months of the transfer. The other required an additional cancellous graft to achieve final union. Refracture of the graft occurred in one patient, but solid and hypertrophic union was reached three months after reapplying external fixator without further cancellous graft. Satisfactory walking was noted in all patients at the last follow-up, although two patients had restricted motion of the knee. |
