A 51-year-old male adult developed acute hepatic failure after hemigas-trectomy performed for a perforated gastric ulcer; the liver condition pre-sented as fever, jaundice and hepatomegaly. The patient had a history of lumbar spine tuberculosis. Percutaneous liver biopsy was done, and the histopathological examination revealed acid fast mycobacterial bacilli and granuloma with caseous necrosis. Hepatic tuberculosis is an uncommon disease, and presenting as miliary type, nodular type or abscess type. Miliary type is the most frequently encountered, and was the diagnosis here. This patient, who had developed hepatic failure caused by hepatic tuberculosis, was successfully treated. In Taiwan, hepatic tuberculosis should be con-sidered in the searching for the cause of hepatic failure in patients who are at risk and have associated jaundice. |
