Eighty three anal fistula patients (58 lower intermuscular, 11 transsphincteric,
and 14 high intermuscular) were studied. The fistula tract was laid open, and various parts of the sphincters were divided. Manometry was done before and 2 months after surgery or after complete healing of the wound. The mean resting pressure (MRP), mean squeeze pressure (MSP), and maximal contraction pressure (MCP) were checked. MRP was decreased by 9% to 35%, with the high intermuscular group showing the largest decrease. The MSP was decreased most in the transsphincteric type, because of divided striated muscle. Incontinence after surgery was also analyzed. There was no relation between incontinence and fistula type or patient's sex, but there was some relationship to age (correlation coefficient 0.2709). |
