Adenocarcinoma of the small intestine is an infrequently encountered tumor with very poor prognosis. The low incidence of tumor oceurrence in the jejunum and ileum leads us to know little about them, we present here the clinical manifestations and the treatment outcome of adenocarcinoma in these areas. Records of 153 patients diagnosed with small bowel malig-nancies that were not periampullary lesions between 1976 and 1996 were reviewed. Excluding those with non-adenocarcinomas (n=75) and duodenal tumors (n=67), 11 cases of primary adenocarcinoma of the small bowel were analyzed. Clinical manifestations, surgical treatment and pathological diagnosis were presented and the surgical outcome was analyzed with Kaplan-Meier method. Among those 1l patients with primary small bowel adenocarcinoma, six had tumors located in the jejunum, and five in the ileum. The mean age was 59.8-7.3 years with a male to female predominance of 2.7:1. Intestinal obstruction was the most common encountered impression (n=7), followed by lower gastrointestinal tract bleeding (n=3). Nine of the 11 patients were diagnosed with roentgen findings of segmental thickness
of the intestinal wall, or "napkin-ring" strictures. Survival rates correlated
well with tumor staging, with a five-year survival rate around 30.7%. We conclude that jejunal and ileal adenocarcinomas are rarely encountered but have poor prognosis. Surgical treatment at an early stage is mandatory to improve the patient's survival. |
