Primary yolk sac tumor (YST) of mediastinum, a rare extragonadal germ
cell neoplasm, primarily inflicts young adult males. This article reports on
three patients with pure mediastinal YST. Initial evaluation consisted of
complete physical examination, imaging studies, determination of serum a-fetoprotein (AFP) and B-human chorionic gonadotropin (B-HCG). An enormous mediastinal mass in the chest films and high serum AFP level were found in all cases. The diagnosis was established by biopsy (biopsy of cervical node, n=1; mediastinotomy with biopsy, n=2) and the histologic examination revealed a typical yolk sac tumor. They were treated with cisplatin-based chemotherapy after diagnosis. Two patients underwent post-chemotherapy thoracotomy with resection of the residual mass. Only one case is currently alive and disease free. An aggressive combined modality approach with cisplatin-based chemotherapy follwed by surgical resection of residual disease is the optimal means of managing this disease. |
