Carcinoid tumors of the alimentary tract are not commonly seen. In
Taiwan, most of these tumors occur in the stomach and rectum. The small
bowel carcinoids (SBC) are very rare. They are either clinically silent or
present with nonspecific signs and symptoms. Precise diagnosis is diffticult
prior to surgery. Small bowel carcinoid tumors may cause mechanical
obstruction even when the primary tumor is small. A 77 year-old male with
no history of abdominal surgery who presented with chronic intermittent
incomplete ileus is described. Repeated physical examination and extensive
gastrointestinal workups during a one-year period failed to result in diagnosis.
Exploratory laparotomy, enterolysis, and segmental resection of the ileum
contains a small indurated mass were performed. Histologic study revealed
a malignant carcinoid tumor in the ileum with no lymph node metastasis. The patient recovered uneventfully after operation and remained well during two years of clinical follow up. Since SBC has a nonspecifie clinical presentation, small bowel obstruction may be the most common sign. For patients with unknown repeated partial ileus, SBC should be considered. |
