Pulmonary mucormycosis is a rare opportunistic disease usually occurring in diabetic or immunocompromised patients. Surgical intervention has been advocated for localized lesions confined to one lung. However, extensive spread of the disease with resulting respiratory failure usually develops in most patients. Management of such conditions has so far been con-troversial. We report a case of pulmonary mucormycosis with bilateral involovement and respiratory failure which was successfully treated by a combination of antifungal medication and surgical resection of the necrotic and cavitary portion of the lesion. It is concluded that an aggressive surgical approach should be considered for pulmonary mucormycosis even with extensive or bilateral involvement.
