Inflammatory pseudotumor (plasma cell granuloma) is a rare benign lesion in adults and children. It frequently simulates a true neoplasm both elinically and morphologically, presenting a diagnostic and therapeutic dilemma. We herein report the case of a 33-year-old man presented with fever, body weight loss, microcytic hypochromie anemia, polyclonal hypergamma-globulinemia, thrombocytosis, and elevation of C-reactive protein. Colono-fiberoscopy showed a space-occupying lesion at the cecum. The patient underwent right hemicolectomy for a presumed colonic neoplasm, and the histologic section revealed that it was an inflammatory pseudotumor. With resection of the mass, there was resolution of
constitutional symptoms and normalization of laboratory abnormalities. Since inflammatory pseudotumors appear to have a self-limiting course and a favorable prognosis, our case and a review of the literature indicate that preoperative or intraoperative recognition is important. The possibility of this entity should be considered if laboratory data on admission suggested an inflammatory process. |
