A case of metastasizing carcinoid of the gastric stump found incidentally 25 years after hemigastrectomy with Billroth II anastomosis for duodenal ulcer bleeding is presented in the present study and the relating literature on carcinoid of stomach is reviewed. The patient had received a regular endoscopic examination and biopsy once a year since 1988. Pre--operatively, there was no evidence of the carcinoid syndrome and the circulating level of gastrin was within the normal range. However, even with aggressive treatment, the patient expired due to carcinomatosis and multiple bone metastases 10 months after the operation. This is the second incidence of the gastric stump carcinoid reported in the literatures. This unusual case shows that gastric carcinoid tumors may develop without trophic effect of hypergastrinemia, and it usually behaves as a high grade malignant tumor. |
