Xanthogranulomatous cholecystits (XGC) is an uncommon, benign, inflammatory process of gall baldder. It is difficult to differentiate from gall baldder cancer both clinically and radiologically, but the principles of management of these two kinds of disease are different. A 67-year-old female was diagnosed as gall bladder tumor combined with cholecystogastric fistula by abdominal computed tomography and upper gastrointestinal barium study. Cholecystectomy, wedge resection of liver bed and gastric antrectomy including the fistula were accomplished followed by gastroduodenal anastomosis and dissection of regional lymph nodes. Pathological examination revealed XGC combined with gall bladder adenocarcinoma and cholecystogastric fistula. The patient recovered uneventfully and was undergoing chemotherapy. There was no evidence of recurrence after one year of follow up. Surgeons should aware that XGC itself is benign but may be misdiagnosed as gall bladder cancer and also there is possibility of coexixtence of both benign and malignant lesions. |
