Intramural duodenal hematoma is rare. The most common cause is blunt abdominal injury. Most cases can be cured with conservative treatment. We describe a case of traumatic intramural duodenal hematoma. After 21 days of conservative treatment, the patient still presented with abdominal pain, vomiting, and jaundice and his direct and total (D/T) serum bilirubin level elevated to 5.7/12.1 mg/dL. Upper gastrointestinal (UGI) series revealed complete obstruction at the first portion of the duodenum. Therefore, surgical removal of the hematioma and a cholecystostomy were performed. After surgery, the D/T serum bilirubin level decreased markedly, jaundice subsided, and gastrointestinal tract obstruction improved. The patient tolerated food intake well and was discharged on the 42nd hospital day without specific complaints. |
