The use of laparoscopic techniques in treating coloectal malignancy is stilla controversial. The aim of this study was to evaluate whrther laparoscopic abdominoperineal resection is feasible. From July 1998 to July 2000, there were 41 lowel rectal cancer patients who received an abdominoperineal resection procedure, 23 patients who received a laparoscopic procedure and 18 patients who received open procedures. Mean ages were 67 and 65 years for the laparoscopic and open procedures, respectively. Sixteen cases of laparoscopic abdominoperineal resection and eleven cases of open abdominoperineal resection were males. One case was converted to an open procedure due to presacral bleeding. Five complications occurred in the laparoscopic group and six in the open group. Mean operative times were linger for the laparoscopic procedures (200 min for laparoscopic procedures vs. 175 min for open procedures; (p<0.05); mean blood loss (250 vs. 410 ml; p<0.05), average bowel function return (2.3 vs 4.1 days; p<0.05), and hospital stay (11 vs. 14 days; p=not significant) were also evaluated. No mortality occurred. The mean follow-up period was 9 months, and no recurrence or distant metastasis was found in the laparoscopic group, but one distant metastasis occurred in the open group. We concluded that laparoscopic abdominoperineal resection is feasivle with irespect to its acceptable complications, ooncological results, operative times, and cost. |
