A 34-year-old woman was seized with right side chest and abdominal pain after a blunt injury during a motor vehicle accident. On arrival at our emergency department, her vital signs were stable. The sonogram revealed a mass over ber right adrenal gland and computed tomography (CT) scan with contrast medium for the abdomen showed a mass lesion around 40 Hounsfield units (HU). Additionally, a patch of intrahepatic hemorrhage was noted in the right lobe of the liver, about 4X2cm. The biochemical test results included: cortisol, aldosterone, vanillylmandelic acid (VMA), sodium and potassium concertration were within reference ranges. Conservative treatment was implemented. Six months after admission, follow-u CT scans revealed complete resolution of the hepatic and the adrenal lesions. Hemorrhages of the adrenal gland had spontaneously resolved. The patient had recovered without any complications. We present this case and review similar reports in the literature. Reports of unilateral adrenal hemorrhage after blunt abodminal injury have been rare. We emphasize the importance of careful inspection of the adrenal glands for patients in whom lower thoracic or upper abdominal injuries are encountered. |
