A case of intracardiac migration of a metallic pin is reported here. A 23-year-old man experienced intermittent onset of chest tightness, retrosternal pain, and pleuritic discomfort worsened with deep inspiration following a heavy duty in the Army. Apart from the operation of open reduction with an internal Steinmann pin fixation due to the medial fracture of his right clavicle fourteen months ago. The patient had no any medical records of cardiac or pulmonary disease. A plain chest radiograph revealed the malposition of a pin over the cardiac silhouette. Echocardiography as well as CT scan showed the retention of metallic foreign body in the left ventricle. A 7-cm-long Steinmann pin penetrating through the root of the main pulmonary artery into the left heart chamber was removed successfully via sternotomy. The postoperative course was uneventful without any complications. The cases reported in the English literature are reviewed to reemphasize the hazards of migration of the metallic devices used for clavicle or shoulder fixation, and all clinicians should be aware of this migration of pin or wire as a potential candidate of complication after the implantation of orthopedic material near thoracic cavity. |
