Acute subdural hematioma (SDH) still carries a high mortality and morbidity injury even nowadays. Many prognostic factors were studied in this communication, including patient's age, neurological function and CT finding before operation, and timing of operation. A total of 72 patients with acute SDH who have been operated by the same neurosurgeon were studied, and 56% had a functional recovery rated good or moderate disablity with the Glasgow outcome scale. Most of our patients underwent operation within 4 hours after trauma. Age, pupillary response, hematoma thickness, and midline shift were in strong correlation with the prognosis of patients having acute SDH. Mesencephalic cisterns condition also had a strong correlation to the prognosis. Of those patients with complete loss of cisterns, only 31% had a functional recovery; of those patients with normal cisterns, only 31% had a functional recovery; of those patients with normal cisterns, 77% had a functional recovery. But subarachnoid hemorrhage (SAH) before operation had no correlation to the prognosis, maybe due to early operation in our series. |
