Endometriosis is a frequent problem in gynecological entity, but rarely seen by general surgeons unless it has extragonadal involvement and requires surgery. The majority of cases with endomeriosis involving bowels manifestated only with cyclic abdominal pain. Here we report a case of endometriosis of the rectum presented with ileus. Polypoid mass under sigmoidoscopy made it indistingushable from carcinoma of the rectum before operation. The final diagnosis was confirmed by ectopic endometrial glands in the resected specimens of ovary, rectum and even pericolonic lymph nodes. Good outcome was obtained by initial sigmoid colostomy and subsequent low anterior resection plus hysterectomy, and bilateral salpingo-oophorectomy. The incidence, symptoms, diagnosis, and treatment of endometriosis invading bowels will be discussed. |
