We would like to share ideas on “Geriatric nutritional risk index (GNRI) in screening malnutrition among young adult and elderly trauma patients.”[1] Liu et al. concluded that “GNRI could serve as a useful screening method to identify high‑risk malnutrition i… but also the young adult trauma patients.”[1] We agree that the GNRI is applicable. Considering components in the formula for calculating GNRI, the body weight and albumin value in young adult or elderly are clinically considered based on the same principle.
If the score is applied for young pediatric group, there should be a problem. Indeed, a previous report already confirmed the applicability of GNRI in various age ranges of patients including adolescents and young adults.[2] However, basic
limitations of GNRI, including hydration status, hormonal status, dependence on laboratory assays, still exist.[3] Due to a possible laboratory aberration due to trauma‑induced blood loss and volume deletion, the albumin level from
measurement should be adjusted by the plasma dilution factor to increase accuracy in the final diagnosis.[4] |
