摘要 Castleman’s disease is a rare disease characterized by either one single hyperplastic lymph node confined to an anatomic site in the unicentric type or generalized lymphadenopathy in the multicentric type. The most common site of Castleman’s disease is the thorax. Castleman’s disease of the pancreas is extremely rare. The treatment for Castleman’s disease of the unicentric type is surgery. In the multicentric type, the treatment remains controversial, and is usually composed of variable chemotherapeutic regimens. We report a case of unicentric Castleman’s disease in the pancreas that was treated successfully by surgery. A 29-year-old female presented with intermittent watery diarrhea for six months. A computed tomography scan revealed one smooth, homogeneous, and well-demarcated tumor in the pancreatic body. The tumor was excised smoothly at laparotomy and the histologic findings were compatible with Castleman’s disease. The postoperative course of the patient was uneventful, with gradual improvement of the symptoms. There was no evidence of recurrence at one-year follow-up. Only six cases of Castleman’s disease in the pancreas have been reported in the English literature. Although rare cases of unresectable pancreatic Castleman’s disease do exist, most patients with unicentric Castleman’s disease of the pancreas can be treated successful by complete excision of the lesion. When total excision can be obtained, the prognosis is excellent.
