Abdominal tuberculosis is rare. Twenty-six patients, 15 with alimentary tuberculosis and 11 with peritoneal tuberculosis, were reviewed in a district general hospital in the south of Taiwan to investigate the clinical features of this disease. A majority of patients had presented with abdominal pain and loss of body weight. Anemia and hypoalbuminemia were the major laboratory abnormalities. Tuberculous lesions in the alimentary tract involved predominantly the ileum (10 patients) and appendix (3 patients) and manifested as intestinal obstruction and acute appendicitis at provisional diagnosis. Peritoneal tuberculosis might be misdiagnosed as acute appendicitis, pyloric stenosis, or lower abdominal mass depending on focal involvement. Seventeen (65.4%) patients were treated with anti-tuberculous therapy only, while the remaining nine patients received operation followed by anti-tuberculous therapy. A majority of patients recovered without complications, and only two patients with alimentary tuberculosis died of sepsis and chronic renal failure postoperatively, respectively. Diagnosis of abdominal tuberculosis is usually difficult and a high index of suspicion is required. It is a condition treatable by chemotherapy and should be considered in the differential diagnosis of other abdominal pathology. |
