Gall bladder injuries after blunt abdominal trauma are rare. A 39-year-old man who was a victim of automobile accident had progressive abdominal distension and pain. The abdominal computed tomography scan revealed abnormal fluid in the gall bldder and pericholic fluid collection. An exploratory laparotomy was performed, and one large perforation of the gall bladder and two liver lacerations were noted during operation. In the past, delay in diagnosis was the rule rather than the exception, and operation was always necessary for making a diagnosis. The use of modern imaging makes preoperative diagnosis of gall bladder injury possible now.
