Situs inversus viscerum (transposition of viscera) is a rare condition with a genetic predisposition that recessive mutation was noted in the inversus viscerum gene which was located in the long arm of chromosome 14, the incidence rate is about 1:20,000 live births. We report 2 cases with situs inversus viscerum that underwent operation, one was left sided appendicitis---who received appendectomy, the other was diagnosed cholelithiasis and choledocholithiasis-who underwent cholecystectomy and cho-ledocholithotomy and T-tube drainage. Both patients were well postopeatively and discharged uneventfully. Operation in patients of situs inversus viscerum could be safely performed, altho-ugh attentions must be paid to the details of left-right reversal.
